YWAM Pneuma Springs

Acknowledgment of Risk, Waiver of Liability, Release of Claims, and Emergency Medical Consent

By signing this agreement, I acknowledge that I have voluntarily chosen to participate in activities, events, classes, seminars, retreats, mission trips, worship gatherings, outreach opportunities, transportation, lodging, meals, and any other programs sponsored or hosted by YWAM Pneuma Springs ("YWAMPS").

1. Assumption of Risk

I understand that participation in these activities involves inherent risks, including but not limited to accidents, slips and falls, transportation incidents, weather-related hazards, illness (including communicable diseases), food-related reactions, physical exertion, and other unforeseen circumstances that may result in injury, disability, property damage, or death.

I voluntarily assume all risks, both known and unknown, associated with my participation.

2. Release and Waiver of Liability

To the fullest extent permitted by law, I release, waive, and forever discharge YWAM Pneuma Springs, its directors, officers, employees, pastors, volunteers, missionaries, agents, representatives, affiliates, and property owners from any and all claims, demands, causes of action, liabilities, damages, costs, or expenses arising out of or related to my participation, including personal injury, illness, property loss, or death, except to the extent caused by gross negligence or willful misconduct where such a release is prohibited by applicable law.

3. Medical Authorization

If I become ill or injured and am unable to provide informed consent, I authorize YWAM Pneuma Springs to obtain emergency medical treatment on my behalf, including transportation by ambulance if deemed necessary.

I understand that I am solely responsible for all medical expenses incurred and that YWAM Pneuma Springs does not provide medical insurance for participants.

4. Health Representation

I certify that I am physically and medically able to participate in these activities or have consulted with my physician regarding my participation. I agree to inform YWAM Pneuma Springs of any medical conditions, allergies, medications, or other information that may be relevant in an emergency.

5. Personal Property

I understand that I am responsible for my own personal belongings and release YWAM Pneuma Springs from liability for lost, stolen, or damaged personal property.

6. Photography and Media

I grant YWAM Pneuma Springs permission to photograph, record, or film me during events and to use these images or recordings for ministry, educational, promotional, or fundraising purposes without compensation, unless I notify YWAM Pneuma Springs in writing before the event.

7. Governing Law

This agreement shall be governed by the laws of the State of Washington without regard to its conflict of law principles.

8. Severability

If any provision of this agreement is determined to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.

9. Acknowledgment

I have carefully read this agreement, fully understand its contents, and understand that I am giving up certain legal rights by signing it.
